UCARe - Person-Specific Information for First Responders    

Have you ever worried about what would happen if a family member or loved one with a disability or other medical condition encountered an emergency? What information would be helpful to emergency responders?
The North Andover Police and Fire Departments are pleased to announce UCARe – a free opportunity for families to provide a uniform method to alert first responders about individuals with special needs. UCARe also provides first responders with advance knowledge, allowing for quick and educated responses during crises, evacuations and other incidents requiring an emergency response.

Who can use it?

Anyone who has a special need, health challenge or disability such as, but not limited to, Alzheimer’s Disease, Autism, Seizure Disorder, Mobility or Cognitive Impairment, or other medical condition, who would like first responders like police, EMTs and fire department to know of their special needs.

Who can fill out the UCARe Form?

Individuals with special needs can fill out the form for themselves
Parents or guardians of minor children
Those with Power of Attorney
Do I have to fill it out?

No, it is VOLUNTARY. You do not have to provide any information you do not want First Responders to know or use.

How do I use the form?

You will need to meet with an authorized representative of the UCARe Team and fill out the form with as much information as you would like to share with emergency responders. A team member will attach a photo of the UCARe individual to the form and this information will be kept in a confidential file, accessible only to emergency responders. The form will be updated annually.

How do I register?

You can register with the nurse at your child’s North Andover Public School.
